Quick answer: Green card applicants filing Form I-485 in the U.S. need an immigration medical exam on Form I-693, done by a USCIS-designated civil surgeon. USCIS requires the completed, sealed I-693 to be submitted with the I-485 in most cases. The COVID-19 vaccine has not been required since January 22, 2025. PACS has civil surgeons who perform walk-in I-693 exams, with required vaccines available on site.
The medical exam is a required step in adjustment of status. Coming prepared saves time and avoids a second visit.

Validity and submission
USCIS requires the sealed I-693 to be filed together with your I-485 in most cases. USCIS ended the old “60-day rule” for civil surgeon signatures in 2023. Rules change, so confirm current requirements with your attorney or at uscis.gov/i-693.
Immigration physicals at PACS
Our civil surgeons perform immigration physicals as walk-ins, with required vaccines available. Patients come from Alexandria, Bailey’s Crossroads, Arlington and across Northern Virginia.
