The sports physical includes a full health history review, vital signs and vision screening, cardiovascular assessment, musculoskeletal exam, and review of any prior injuries or concerns. We discuss safe training practices and complete all required clearance forms.

Bring the sports/league required form, your athlete’s immunization record, and a list of any medications or known conditions (including asthma, allergies, and prior concussions). Visit time is typically 20–30 minutes. You leave with a signed clearance form.
