Quick answer: Virginia High School League (VHSL) athletes need a completed VHSL Pre-Participation Physical Examination (PPE) form before they can practice or play. Under the current VHSL rule, a physical is valid for 14 months from the date the provider signs it. Your school may require specific pages, so check with your athletic department. PACS offers walk-in sports physicals for $55 self-pay at both clinics.
Tryouts and preseason practices come fast. Getting the physical done early avoids missing the first day.

What the VHSL form includes
- A health history completed by the student and parent
- A physical exam completed by the provider
- A clearance section signed by the provider
Download the current form from your school’s athletics page. Bring all pages, with the history section filled out, to the appointment.
How long is a VHSL physical valid?
The VHSL now uses a 14-month window from the signature date. Older rules tied validity to a May 1 cutoff, so if you see conflicting information, follow your school’s current athletic department guidance.
