Quick answer: Virginia requires a comprehensive physical exam for children entering public kindergarten or elementary school for the first time, completed within 12 months before school entry. It’s documented on the School Entrance Health Form (MCH 213G), which has three parts: health history (completed by the parent), immunization certification, and the physical exam report. A licensed physician, nurse practitioner or physician assistant can complete it, including at a walk-in clinic like PACS.
The form causes a lot of last-minute stress for parents every August. Here’s what to know so your child’s paperwork is ready on time.

Who needs the form?
- Children entering public kindergarten in Virginia
- Children entering a Virginia public elementary school for the first time, such as a family moving from another state
Middle and high school students entering Virginia public schools don’t need a school entrance physical, though they need up-to-date immunization records. Students who repeat kindergarten don’t need a second exam.
The 12-month rule
The exam must be done within 12 months before your child first starts school. A physical from a pre-K year only counts if it falls inside that 12-month window.
The three parts of MCH 213G
- Part I, Health Information: filled out by the parent or guardian, then reviewed by the examining provider.
- Part II, Certification of Immunization: completed by a medical provider.
- Part III, Comprehensive Physical Examination Report: completed by the examining provider.

How to get it done at PACS
PACS offers walk-in school physicals at both clinics. Bring:
- The MCH 213G form, with Part I already filled out
- Your child’s immunization record
- Insurance card, or use self-pay pricing ($95 for a school physical)
If your child is missing vaccines, your pediatrician or local health department can help bring them up to date. See our full back-to-school checklist.
Frequently asked questions
Can urgent care fill out the Virginia school physical form?
Yes. Virginia allows licensed physicians, nurse practitioners and physician assistants, including at walk-in clinics, to complete the exam.
Can I use a different form?
Some schools accept another physical report if it covers the same information. The school nurse decides, so the MCH 213G is the safest choice.
Does my child need a new physical for sports?
Yes. High school sports require the VHSL form. See VHSL sports physicals.
